HAZOP studies can fail even when experienced technical people are in the room. Participants learn the full HAZOP leadership workflow: setting study objectives and scope; selecting the team; preparing P&IDs, process descriptions and operating envelopes; defining nodes and design intent; applying parameters and guidewords; recording deviations, causes, consequences, safeguards, recommendations and action owners. The course connects HAZOP findings to HSE risk management through risk matrices, barrier thinking, ALARP judgement, action prioritisation, management of change and close-out assurance. Participants also practise facilitation techniques for maintaining challenge, managing disagreement and preventing dominant voices from distorting the study.

Teaching combines instructor-led technical sessions with facilitated HAZOP simulations based on realistic process and utility scenarios. Teams work from simplified P&IDs, operating information and hazard registers, using structured worksheets and PHA software-style records. Each participant leaves with a completed HAZOP leadership pack: a study preparation checklist, node-definition template, guideword worksheet, action-register format, meeting facilitation plan and personal application plan for an upcoming or representative workplace study.

Participants do not need previous HAZOP chairing experience, formal process safety certification, advanced calculations or specialist software expertise. This is a foundation-to-intermediate course: complete beginners should expect to learn HAZOP terminology and structure before progressing to facilitated node reviews, while experienced team members will focus on leadership, recording quality and action governance.

Training methodology

The course uses short instructor-led modules followed by progressive HAZOP workshops. Participants interpret P&IDs and operating narratives, define nodes, test design intent with guidewords, and record findings in structured worksheets and PHA-Pro-style fields. Case discussions examine weak recommendations, missing safeguards, chairing challenges and action close-out failures. Rotating roles give participants practice as chair, scribe, process engineer and operations representative. On the final day, each participant prepares an application plan and leadership pack for a forthcoming or representative workplace HAZOP.

Course outline

Day 1: HAZOP foundations and study preparation

  • Purpose of HAZOP within HSE and process safety risk management
  • IEC 61882 principles and HAZOP study terminology
  • Relationship between HAZOP, HIRA, LOPA, BowTie and management of change
  • Study objectives, scope boundaries and success criteria
  • Multidisciplinary team roles, independence and competence requirements
  • Document readiness review for P&IDs, narratives, procedures and design data
  • HAZOP study plan, agenda, logistics and preparation checklist

Workshop: Participants review a process modification pack and produce a HAZOP scope statement, document register and team-role plan.

Day 2: Nodes, design intent and guideword analysis

  • Selecting nodes using process boundaries, equipment functions and control loops
  • Writing clear design intent and normal operating conditions
  • Process parameters including flow, pressure, temperature, level, composition and time
  • Guidewords including no, more, less, reverse, other than, early and late
  • Constructing meaningful deviations from parameter and guideword combinations
  • Screening non-credible deviations without suppressing valid concerns
  • Using P&IDs and operating envelopes during node-by-node review

Workshop: Teams define nodes and design intent for a simplified transfer and storage system, then create a deviation worksheet for the first node.

Day 3: Causes, consequences and safeguards

  • Identifying initiating causes across equipment, control, human and external factors
  • Tracing consequences to personnel, environment, assets and business interruption
  • Differentiating prevention safeguards from mitigation safeguards
  • Testing safeguard independence, effectiveness, availability and survivability
  • Human factors, alarm response, procedures and operator workload
  • Environmental release pathways and containment considerations
  • Recording causes, consequences and safeguards in a disciplined HAZOP worksheet

Workshop: Participants conduct a facilitated HAZOP review of two nodes and produce completed cause-consequence-safeguard records.

Day 4: Risk decisions and HAZOP leadership

  • Applying corporate risk matrices to HAZOP findings
  • ALARP judgement and proportionate risk-reduction measures
  • Escalating scenarios for LOPA, SIL assessment, QRA or specialist review
  • Writing recommendations with clear action verbs and acceptance criteria
  • Facilitating challenge, disagreement and technical uncertainty
  • Managing time, participation, digressions and dominant contributors
  • Scribe-chair coordination and live quality checks in PHA-Pro

Workshop: In rotating chair and scribe roles, participants lead a timed HAZOP session and produce prioritised recommendations with action owners.

Day 5: Reporting, close-out and workplace application

  • Recommendation close-out before design approval, commissioning or operational change
