Course overview

Incident data often arrives through inconsistent forms, incomplete witness statements, email chains and spreadsheets that cannot show whether corrective actions are actually preventing recurrence. Safety professionals need a defensible process for recording events, classifying severity, investigating root causes, assigning actions and presenting reliable trends to leaders. This course focuses on using Intelex Incident Management to create that process, from first notification through investigation closure and management reporting.

Participants work through the Intelex EHSQ platform workflow for incidents, near misses, hazards, observations and occupational injury or illness cases. They learn to configure and use incident forms, classifications, notifications, investigations, action plans, approvals, dashboards and reports. Practical skills include applying consequence and likelihood criteria, using 5 Whys and cause-and-effect analysis, setting corrective-action due dates and escalation rules, validating closure evidence, and producing trend reports that support prevention decisions.

The course is delivered as a five-day instructor-led workshop in a classroom or live online environment, using realistic incident scenarios and guided exercises in an Intelex training environment. Participants complete a capstone case that follows a workplace event from initial report to closed investigation and executive dashboard. They leave with an incident workflow map, an investigation and action-plan template set, a configured reporting concept, and a 90-day application plan for improving their own organisation's incident-management process.

Requirements and prerequisites

Participants should understand basic occupational health and safety terms such as hazard, incident, near miss, corrective action, root cause and risk assessment. Experience reporting or investigating workplace events is helpful but not essential. Familiarity with web-based business applications, spreadsheets and simple filtering is assumed, because participants will enter records and interpret dashboard outputs. Access to an organisational Intelex instance is not required for attendance; a training environment can be used. No programming, database administration, advanced analytics or prior Intelex configuration experience is required. Complete beginners should expect a structured introduction before moving into workflow and reporting exercises.

Training methodology

An experienced instructor combines short demonstrations in Intelex with guided record-entry, investigation and reporting exercises. Participants work from a realistic multi-site incident case, progressing from notification and classification through evidence capture, root-cause analysis, action assignment and closure review. Small-group workshops test how routing, escalation and approval choices affect accountability. Each day includes a practical output that is reviewed against safety-management requirements. The final session uses peer critique and instructor coaching to turn the completed case into a site-specific implementation and reporting plan.

Workshop: Participants map their current reporting process against an Intelex incident lifecycle and produce a gap list of fields, roles and hand-offs.

Day 2: Reporting and triaging workplace events

• Creating incident reports through forms, mobile-ready fields and attachments
• Designing mandatory fields and conditional questions for complete intake records
• Capturing people, locations, equipment, contractors and environmental factors
• Recording injury and illness details, treatment outcomes and lost-time information
