Course overview
Late or incomplete incident records weaken corrective action, distort safety metrics, and make it difficult for HSE leaders to demonstrate control of occupational health and safety risks. This course addresses the practical challenge of using Cority to capture incidents consistently, route them to the right investigators, preserve evidence, assign actions, and produce defensible reports. Participants learn how reporting choices affect record quality, regulatory outputs, trend analysis, and management decisions.
The course covers the Cority Incident Management workflow from initial notification through closure. Participants practise creating and classifying incidents, near misses, hazards, injuries, environmental events, and property damage records; applying severity and potential-risk criteria; managing witness statements and attachments; conducting investigations with root-cause methods; and tracking corrective and preventive actions. They also learn to use role-based workflows, approval stages, overdue-action controls, incident registers, dashboards, and scheduled reporting to support ISO 45001-aligned HSE management.
Delivered through instructor demonstrations, guided Cority sandbox exercises, and realistic HSE case files, the programme focuses on decisions users must make in live reporting situations. Each participant builds an Incident Reporting Operating Playbook containing a reporting workflow map, classification rules, investigation checklist, action-escalation matrix, and dashboard requirements for their own site or business unit. This provides a practical reference for improving adoption and reporting discipline after the course.

Requirements and prerequisites
Participants should understand their organisation’s basic incident, near-miss, hazard, and corrective-action process and have experience using web-based business systems. Familiarity with common HSE terms such as injury classification, root cause, risk rating, corrective action, and investigation is assumed. Prior use of Cority is helpful but not mandatory; the course introduces the relevant Incident Management screens and workflow logic in a training environment. Participants do not need programming, database administration, report-writing code, or advanced system-configuration experience. Those without any HSE reporting experience should first gain familiarity with their organisation’s incident procedure.
Training methodology
The instructor uses a Cority training environment to demonstrate each stage of the incident lifecycle, then participants complete guided record-entry, investigation, action-tracking, and reporting tasks. Case studies include a lost-time injury, a high-potential near miss, and an environmental release, requiring teams to agree classifications, investigation scope, and escalation decisions. Short review sessions connect Cority fields and workflows to ISO 45001 controls. On the final day, participants convert their learning into a site-specific Incident Reporting Operating Playbook and implementation priorities.

Day 1: Cority incident reporting foundations

- Incident lifecycle from notification through verified closure
- Incident, near-miss, hazard, injury, and environmental event record types
- Required fields, conditional fields, and data-quality controls
- Location, organisational hierarchy, employee, and contractor data selection
- Attachments, photographs, witness statements, and evidence traceability
- Draft, submitted, reviewed, approved, and closed record statuses
Workshop: Participants create and submit a complete Cority near-miss record from a construction-site case file, including people, location, evidence, and initial controls.
Day 2: Classification, severity, and notification
- Incident taxonomy design and consistent event classification
- Actual severity versus potential severity assessment
- Injury and illness classification for HSE reporting
- Hazard source, energy type, and exposure categorisation
- Risk matrix application for initial event prioritisation
- Management notification triggers and escalation thresholds

Workshop: Participants classify a mixed set of injury, environmental, property-damage, and near-miss cases and build a decision table for routing and notification.
Day 3: Investigation and causal analysis in Cority

- Causal-factor analysis for multi-step operational events
- Immediate causes, contributing factors, and systemic control failures

Workshop: Teams investigate a forklift-pedestrian collision scenario in Cority and produce an evidence-backed causal analysis with documented findings.
Day 4: Corrective actions and performance reporting
