Course overview
Environmental incidents demand more than a site clean-up and a brief report. HSE officers must establish what happened, preserve defensible evidence, identify failed controls, assess regulatory and environmental consequences, and recommend actions that prevent recurrence. Weak investigations can lead to repeated spills, inaccurate regulator notifications, ineffective corrective actions, damaged stakeholder trust, and avoidable enforcement exposure. This course equips HSE officers to lead environmental investigations that stand up to internal review, regulator scrutiny, and management decision-making.
Participants learn a structured investigation process for releases, spills, permit breaches, waste incidents, emissions excursions, contamination discoveries, and near misses. The course covers scene management, evidence control, interviewing, chronology building, causal analysis, environmental impact assessment, notification decisions, corrective and preventive action (CAPA), and report writing. Participants use methods including event-and-causal-factor charting, the 5 Whys, barrier analysis, fishbone diagrams, and risk-based action prioritisation.
Delivery combines instructor-led briefings with realistic incident files, photographs, sampling records, witness statements, permit conditions, and operational data. Teams investigate a simulated environmental release from initial notification through to management review. Each participant leaves with a completed investigation pack: incident classification record, evidence log, causal analysis, corrective-action register, regulator-ready report structure, and a workplace action plan for improving their organisation's investigation process.
The programme is suited to HSE officers who investigate or support environmental incidents in industrial, construction, energy, utilities, logistics, waste, manufacturing, and facilities operations. It also gives managers confidence that incident findings will translate into accountable, risk-based improvements rather than generic recommendations.

Requirements and prerequisites
Participants should be familiar with their organisation's basic HSE procedures, incident reporting route, and the environmental aspects of their operations, such as waste, emissions, chemicals, discharges, or land use. Experience of completing incident reports, inspections, or risk assessments is helpful but not essential. Learners should be comfortable reading procedures, permits, simple monitoring records, and spreadsheet tables. A working knowledge of local environmental legislation is not assumed; jurisdiction-specific notification duties are addressed as a decision-making framework rather than legal advice. No previous root-cause-analysis qualification, sampling qualification, or specialist investigation software experience is required.
Training methodology
The course uses short instructor-led modules followed by practical investigation work. Participants examine a progressive case file containing incident notifications, photographs, permits, monitoring results, maintenance records, and witness accounts. In groups, they protect the evidence trail, interview role-play witnesses, map the event sequence, test causal hypotheses, and build a corrective-action register. Facilitated peer review challenges unsupported conclusions and vague actions. On the final day, participants convert the completed case into a report and create an application plan for their own reporting and investigation arrangements.

Day 1: Environmental incident fundamentals and first response
- Environmental incident, near-miss, nonconformance, and emergency classifications
- Legal, permit, contractual, and internal reporting triggers
- Source-pathway-receptor model for environmental harm
- Initial severity screening and receptor sensitivity assessment
- Scene safety, containment, escalation, and protection of evidence
- Incident notification logs and first-response information capture
- Roles, authority, and independence within an investigation team
Workshop: Participants triage three incident notifications and produce an incident classification, escalation decision, and first-response evidence checklist.
Day 2: Evidence collection and fact finding
- Investigation planning using scope, objectives, boundaries, and lines of enquiry
- Evidence logs, exhibit numbering, and chain-of-custody principles
- Photographic evidence, site sketches, and location mapping
- Document review of permits, procedures, manifests, inspection records, and maintenance data
- Environmental monitoring, sampling records, and data-quality limitations
- Structured witness interview planning and open-question techniques
- Chronology building from conflicting accounts and time-stamped records
Workshop: Teams prepare an investigation plan, evidence register, interview guide, and verified event chronology from a simulated chemical release file.
Day 3: Causal analysis and failed-control evaluation

- Fishbone diagrams for people, plant, process, materials, environment, and management factors
- Barrier analysis of preventive and mitigative controls

Day 4: Impact evaluation, reporting, and corrective action

- Significance evaluation using extent, duration, reversibility, and sensitivity criteria

- Corrective, preventive, and improvement action design
- Risk-based CAPA prioritisation using likelihood, consequence, and control strength

- Investigation report structure, findings language, and executive summaries
Workshop: Teams draft a risk-ranked CAPA register and an executive incident report summary for management review.
Day 5: Investigation close-out and workplace application
