Course overview
Healthcare employers must balance competitive pay, clinician retention, affordability, patient-service coverage, and a tightly regulated compensation environment. HR and rewards professionals need to administer base pay, differentials, incentive plans, benefits eligibility, leave, and executive or physician arrangements without creating inequities, payroll errors, benefit-plan exposure, or fair-market-value concerns. This course addresses the operational decisions behind compensation and benefits administration in hospitals, health systems, clinics, long-term care providers, and healthcare support organisations.
Participants learn to analyse healthcare job structures; administer salary ranges, shift differentials, call pay, premiums, bonuses, and incentive plans; and manage benefits enrolment, eligibility, life-event changes, leave, and retirement-plan processes. The programme covers workforce pay data, market pricing, pay-equity analysis, FLSA classification, ACA eligibility measurement, ERISA plan administration, COBRA events, HIPAA privacy controls, and the compensation governance required for physician and senior-leader arrangements. Participants also practise documenting decisions, reconciling HRIS and payroll data, and communicating pay and benefits policies to managers and employees.
Instructor-led briefings are paired with healthcare case files, compensation worksheets, HRIS-style transactions, policy reviews, and small-group decision workshops. Participants work through scenarios involving nursing differentials, clinical incentive payments, benefits eligibility disputes, compensation exceptions, and audit findings. They leave with a practical Healthcare Compensation and Benefits Administration Playbook containing a compensation-calculation template, benefits-administration checklist, compliance calendar, approval workflow, audit controls, and 90-day implementation plan.
The course is designed for experienced HR, rewards, payroll, benefits, and workforce leaders who already support employee populations and now need stronger healthcare-specific administration and governance capability.
Course objectives
By the end of this course, participants will be able to:
- Analyse healthcare job architecture using job descriptions, FLSA status, grade structures, and market-pricing data
- Calculate base-pay adjustments, shift differentials, call pay, overtime impacts, and premium-pay scenarios
- Administer clinical incentive and retention plans using eligibility rules, payout controls, and approval documentation
- Apply ACA, ERISA, COBRA, HIPAA, and leave-administration requirements to common benefits events
- Build a benefits eligibility and life-event workflow for hires, status changes, leaves, and terminations
- Conduct a pay-equity review using regression-ready data fields, comparator groups, and documented remediation actions
- Evaluate physician and executive compensation arrangements against fair-market-value, commercial-reasonableness, and governance requirements
- Produce a healthcare compensation and benefits administration playbook with controls, calendars, templates, and escalation paths
Benefits of attending
For you
- Gain confidence calculating and defending clinical pay elements such as differentials, call pay, premiums, and retention awards

- Learn to identify compliance-sensitive benefits and compensation events before they become payroll or audit issues
- Strengthen credibility with nursing, clinical, finance, payroll, and executive stakeholders through consistent decision documentation

For your organisation
- Reduce payroll corrections and employee disputes through clearer administration of differentials, premiums, incentives, and deductions

- Lower benefits compliance exposure through stronger ACA, COBRA, ERISA, HIPAA, and eligibility-event workflows
- Create a more defensible audit trail for compensation exceptions, physician arrangements, and benefits-plan administration
- Increase consistency across sites and departments with standard checklists, approval matrices, and compliance calendars
Target competencies
Who should attend
- Healthcare Compensation Managers — who administer salary structures, clinical premiums, incentive plans, and pay exceptions
- Benefits Managers — who manage eligibility, enrolment, vendor administration, compliance events, and employee queries
- HR Business Partners — who advise operational leaders on pay decisions, staffing premiums, and benefits-policy application
- Healthcare Payroll Managers — who must translate complex pay rules and benefit deductions into accurate payroll processing
- Total Rewards Specialists — who support market pricing, pay-equity reviews, plan design, and rewards governance
- HR Directors and Workforce Leaders — who approve compensation controls and need defensible workforce-cost decisions
Requirements and prerequisites
Participants should have practical experience in HR, payroll, benefits, compensation, or workforce administration, ideally within a healthcare provider, payer, clinic, care-home, or related organisation. They should understand basic payroll concepts, employee eligibility records, salary ranges, benefit enrolment, and common HRIS transactions. Familiarity with Excel formulas, filters, and tables is assumed because participants will work with compensation and eligibility data. Prior legal training is not required, and participants do not need to be experts in ERISA, ACA, HIPAA, FLSA, or physician compensation regulation; the course explains how to apply these requirements operationally and when to escalate issues to legal counsel.
Training methodology
The five-day programme combines instructor-led technical sessions with healthcare-specific calculations, policy diagnostics, and operational casework. Participants use sample workforce files to price jobs, calculate nursing and on-call premiums, test benefits eligibility, investigate payroll variances, and document compensation exceptions. Small groups assess realistic hospital and clinic scenarios involving staffing shortages, leave changes, enrolment disputes, and physician compensation governance. Each day closes with an applied exercise, while the final day converts learning into a site-ready administration playbook and 90-day action plan.
