Course overview
HSE incidents are rarely caused by a single unsafe act or equipment failure. When investigations stop at worker error, organisations miss the failed controls, design decisions, supervision gaps, maintenance weaknesses and organisational conditions that allowed the event to occur. This five-day ICAM HSE Incident Investigation and Learning Training Course prepares experienced safety professionals to investigate serious incidents with sufficient rigour to produce defensible findings, practical corrective actions and learning that prevents recurrence.
Participants apply the Incident Cause Analysis Method (ICAM) to define the event, preserve evidence, build a chronology, identify absent or failed defences, and analyse individual and organisational factors. The course develops practical skills in interviewing witnesses, testing evidence, mapping causal relationships, distinguishing root causes from contributing factors, and developing SMART corrective actions. Participants also learn how to write investigation reports that separate verified facts from assumptions and communicate findings to operational leaders without assigning premature blame.
Delivery combines instructor-led ICAM demonstrations with realistic HSE case files, evidence-review workshops, interview simulations and facilitated investigation-team exercises. Throughout the week, participants work through a complete incident scenario involving operational, environmental and management-system issues. They leave with an ICAM investigation pack containing an event chronology, evidence register, barrier analysis, ICAM causal-factor map, findings, corrective-action register and a workplace application plan.
The course is suited to practitioners who already participate in incident reviews and now need a structured method for leading or quality-assuring investigations into high-potential events, injuries, process safety events, environmental releases and repeated operational failures.

Requirements and prerequisites
Participants should have practical exposure to workplace incident reporting, risk assessment and HSE management systems, ideally through an HSE, operational, engineering or supervisory role. They should understand basic concepts such as hazards, controls, corrective actions, evidence and legal or internal reporting obligations. Prior experience of participating in an investigation is useful but not essential. Participants do not need previous ICAM training, formal forensic qualifications, specialist software expertise or advanced statistics. A laptop with spreadsheet and document-editing capability is recommended for the case exercises and for adapting the investigation templates.
Training methodology
The programme uses short instructor-led explanations followed by progressive application to a realistic HSE incident case. Participants examine an evidence bundle, establish facts, build a chronology, plan witness interviews and create an ICAM map in small investigation teams. Facilitated peer review challenges causal claims, evidence quality and proposed actions in the same way an internal investigation panel would. The final day includes a report-writing workshop and an individual application plan identifying how participants will use ICAM templates, roles and review points within their own investigation process.

Day 2: Evidence collection and event reconstruction

Day 3: ICAM causal analysis and barrier failures
