Course overview
Healthcare quality governance fails when boards receive activity data without assurance, improvement projects operate outside strategic priorities, and serious incidents are treated as isolated events rather than signals of system weakness. Senior quality professionals need to turn patient safety, clinical effectiveness, experience, workforce and compliance evidence into decisions that leaders can defend. This course addresses the practical challenge of designing governance arrangements that identify deteriorating performance early, assign clear accountability, test whether controls work and sustain measurable improvement across services.
Participants examine advanced quality governance models, board assurance frameworks, quality strategy deployment and escalation routes. They learn to define quality domains and metrics, build balanced scorecards, interpret statistical process control charts, apply risk-based assurance, conduct root cause analysis and failure mode and effects analysis, and link incident learning to corrective action. The programme also covers committee terms of reference, quality impact assessment, audit planning, regulatory evidence and reporting approaches aligned to ISO 7101:2023 and the IHI Model for Improvement.
Instructor-led teaching is built around realistic hospital, community and social care scenarios: a rising harm rate, an overdue action plan, inconsistent clinical audit results and a board seeking assurance on a quality objective. Participants work with mock dashboards, incident records, audit findings and risk registers, then challenge evidence as a quality committee would. Each participant leaves with a tailored Quality Governance Assurance Pack containing a governance map, board-level metric set, escalation thresholds, assurance questions, risk controls and a 90-day implementation plan for their own organisation.
The course is suited to experienced quality, safety, clinical governance and operational leaders who already contribute to improvement or assurance activity and now need to shape the systems through which quality is governed.
Course objectives
By the end of this course, participants will be able to:
- Design a quality governance architecture linking board oversight, executive accountability, operational forums and frontline improvement work
- Construct a board-level quality dashboard with outcome, process, balancing and equity measures
- Interpret run charts and statistical process control charts to distinguish common-cause from special-cause variation
- Apply a board assurance framework to test controls, evidence sources, gaps and residual quality risks
- Facilitate root cause analysis and failure mode and effects analysis for complex patient safety and care-quality failures
- Set risk-based escalation thresholds, committee reporting routes and action-tracking requirements
- Evaluate clinical audit, patient experience and incident data for reliability, triangulation and regulatory assurance
- Produce a Quality Governance Assurance Pack and 90-day implementation plan for a live organisational priority
Benefits of attending
For you
- Gain the confidence to challenge whether a board paper provides assurance rather than merely describing activity
- Build a portfolio-ready Quality Governance Assurance Pack based on a real service priority
- Develop credible fluency in SPC, risk-based assurance and quality metric design for senior-level discussions
- Strengthen readiness for director-level quality, patient safety, governance and transformation roles
- Learn to translate incident, audit and patient experience findings into accountable actions with measurable controls
For your organisation
- Creates clearer lines of accountability from board quality objectives to operational action owners
- Improves early detection of deteriorating quality through better thresholds, trend analysis and escalation criteria
- Reduces duplicated reporting by aligning dashboards, committee packs, audit evidence and risk registers
- Produces more defensible regulatory and board assurance through structured evidence testing and control reviews
- Accelerates closure of recurring safety and quality issues by linking investigations to monitored corrective actions
Target competencies
Who should attend
- Directors of Quality and Governance — who need defensible assurance arrangements for boards and regulators
- Clinical Governance Leads — who coordinate committees, audits, incident learning and quality reporting
- Patient Safety Managers — who must convert safety events and investigations into system-level controls
- Senior Nurses and Allied Health Professional Leaders — who lead quality standards and improvement across clinical services
- Healthcare Operations Managers — who are accountable for performance, risk escalation and delivery of corrective actions
- Quality Improvement Programme Leads — who need to connect improvement portfolios to executive governance and assurance
Requirements and prerequisites
Participants should have practical experience of healthcare quality, patient safety, clinical governance, service improvement or operational management. They should already understand basic quality measures, risk registers, incident reporting, clinical audit and the Plan-Do-Study-Act cycle, and be comfortable reading spreadsheets and management dashboards. Experience attending governance meetings or contributing to quality reports is strongly beneficial. Participants should bring, where organisational policy permits, a non-confidential example of a current quality priority, dashboard or risk. No prior certification in Lean, Six Sigma, ISO standards, Minitab or Power BI is required; the course explains the applied use of these approaches.
Training methodology
The programme combines instructor-led briefings with facilitated analysis of quality committee papers, incident records, audit extracts and dashboard data. Participants use Minitab-style SPC outputs and Power BI dashboard examples to identify misleading measures, variation signals and assurance gaps. Small groups conduct a mock quality committee review, test a risk-control chain and prioritise corrective actions after a simulated care failure. Daily application clinics allow participants to adapt the methods to their own setting. On day five, each participant assembles and peer-reviews a Quality Governance Assurance Pack and implementation plan.
Course outline
Day 1: Quality governance architecture and accountability
- Quality governance purpose, scope and operating principles
- Board, executive, committee and service-line accountability mapping
- Three lines model for healthcare quality assurance
- Terms of reference, decision rights and meeting cadence
- Quality strategy deployment through objectives and key results
- ISO 7101:2023 quality management requirements
- Governance failure modes and assurance gap diagnosis
Workshop: Participants map their current or case-study governance structure and produce an accountability and escalation map showing decision rights, evidence flows and gaps.
Day 2: Measurement, dashboards and variation
- Outcome, process, balancing and equity measure selection
- Operational definitions and data-quality testing
- Metric hierarchies from ward or team to board
- Run charts and statistical process control rules
- Common-cause and special-cause variation interpretation
- Power BI dashboard design for quality governance
- Thresholds, targets and exception-reporting logic
Workshop: Using a simulated quality dataset, participants build a board dashboard specification and write an exception narrative supported by SPC evidence.
Day 3: Patient safety, risk and learning systems
- Patient safety incident response and systems-based learning
- Root cause analysis evidence collection and causal factor mapping
- Failure mode and effects analysis scoring and prioritisation
- Risk registers, risk appetite and residual-risk statements

Workshop: Teams investigate a simulated medication-harm event, complete a causal analysis and produce a prioritised control and action plan.
Day 4: Assurance, audit and regulatory readiness
- Board assurance framework construction
- Triangulating incident, audit, complaints and experience evidence
- Risk-based clinical audit planning
- Quality impact assessment for service change
- Internal assurance reviews and evidence sampling
- Regulatory evidence trails and document control
